About The Position

The Volunteer and Bereavement Coordinator is an integral member of the hospice team and assumes responsibility for recruitment, coordination, provision and program administration of all hospice volunteer and bereavement services in conjunction with regulatory standards and DHHC policies. This job description is not meant to be exhaustive and may be modified as needed. Employees may be assigned other related duties to meet organizational needs.

Requirements

  • Associate’s degree in social work, Psychology, Counseling, Human Services, Healthcare Administration, Education, or a related field or an equivalent combination of education and experience.
  • Minimum of 2 years of experience in hospice, healthcare, volunteer management, bereavement services, or a related human services field.
  • Knowledge of hospice philosophy, end-of-life care, grief and bereavement principles, and patient/family-centered care.
  • Strong organizational, communication, and interpersonal skills with the ability to work compassionately with patients, families, volunteers, and interdisciplinary teams.
  • Proficiency with Microsoft Office and electronic health records or volunteer management databases.
  • Valid driver's license, reliable transportation, and the ability to travel throughout the service area.
  • Must have current car insurance , which meets minimum standards.
  • BLS within 90 days of hire

Responsibilities

  • Lead and continuously improve the Hospice Volunteer and Bereavement Programs by assessing program needs, developing new services, and ensuring volunteer and bereavement support aligns with organizational and community needs.
  • Recruit, onboard, train, and retain a diverse, engaged volunteer workforce that meets hospice patient, family, and organizational needs while ensuring all regulatory training requirements are completed.
  • Provide ongoing supervision, coaching, and performance management for volunteers, ensuring volunteers are supported, recognized, and effectively matched to patient and organizational needs.
  • Manage volunteer program compliance and documentation, maintaining accurate records for HR requirements, competencies, education, patient assignments, service hours, and regulatory standards.
  • Direct a comprehensive bereavement program from admission through 13 months following a patient's death, ensuring individualized support for families and caregivers.
  • Collaborate with the Hospice Interdisciplinary Group (IDG) to integrate volunteer and bereavement services into patient care plans and coordinate volunteer assignments based on patient needs and volunteer skills.
  • Evaluate program effectiveness and drive continuous quality improvement by monitoring volunteer utilization, bereavement outcomes, and service quality, implementing improvements as needed.
  • Promote hospice and bereavement services within the community by supporting outreach, education, presentations, and volunteer engagement while ensuring policies and procedures remain current and compliant.
