Requirements

  • Current LVN/LPN licensure (in good standing) in the state(s) of practice with one year of experience under license.
  • Current CPR Certification.
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
  • Ability to learn an electronic medical records system.
  • Reliable Transportation, Driver's License, Car insurance.

Nice To Haves

  • Previous hospice experience preferred.

Responsibilities

  • Assist patients' needs over the phone, providing medical advice within scope of practice.
  • Coordinate care and determine the urgency of patients' situations to ensure appropriate care.
  • Offer support and guidance to patients and their families during difficult times.
  • Deliver telephonic nurse assessment, advice, and instruction to patients and caregivers after normal operating hours within scope of practice.
  • Provide information to the case manager and management team regarding changes in patient condition after telephonic contact.
  • Communicate health care instructions to the patient/caregiver clearly and concisely.
  • Teach the patient and family/caregiver self-care techniques as appropriate.
  • Communicate with the physician regarding patient needs and report changes in patient condition; obtain/receive physician's orders as required.
  • Assign nursing visits accordingly.
