About The Position
The Hospice Team Leader oversees a team of Registered Nurse Case Managers (RNCMs), Social Workers and Chaplains. The position plans, organizes and directs hospice care with emphasis on community health education and experience. The Hospice Team Leader builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Requirements
- Current RN license
- Minimum of 3 years of hospice with supervisory experience
- Valid driver's license and insured, reliable vehicle
Responsibilities
- Enhance communication to ensure existing RN Case Managers, Social Workers and Chaplains receive support in the field.
- Conduct weekly IDT meetings for their designated team.
- Guide new staff with new employee orientation and education in hospice care.
- Perform timely performance check-ins with their team on a quarterly basis.
- Participate in ride-a-long visits with their team on a quarterly basis and as needed.
- Maintain a small caseload.
- Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
- Participate in on call coverage.
