About The Position

The Hospice Team Coordinator plays a crucial role in coordinating and overseeing the delivery of hospice care services to patients. Requirements

  • High school diploma or GED required.
  • At least two (2) years of experience with medical records, medical office, or business office support highly preferred.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
  • Strong written and verbal communications.
  • Ability to understand, read, write, and speak English.
  • Articulates and embraces integrated healthcare at home philosophy.
  • Effective communication with physicians, nurses, and other healthcare personnel.
  • Experienced in answering telephones and responding to patients or families.
  • High integrity, including maintenance of confidential information such as patient records.

  • Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.

Responsibilities

  • Accurately enter and manage patient data, including referral intake, new admissions, and demographic updates.
  • Ensure all admission documentation is complete, signed, dated, and includes the attending physician/medical director's CTI and order to admit, uploading these into the HCHB system.
  • Use HCHB to maintain and update patient records, generate reports for regulatory compliance, and add coordination notes.
  • Monitor durable medical equipment (DME) and medical supply orders, reporting any trends or issues to management.
  • Monitor the timely return of physician orders and facilitate their processing.
  • Verify and upload paper-based visit records into the EMR and HCHB systems, ensuring accurate patient histories.
  • Organize uploaded documents systematically in patient charts for easy retrieval.
  • Confirm that the ordered level of care matches the billable level of care for accurate billing and reimbursement.
  • Collect and upload decertification paperwork into patient records.
  • Notify relevant healthcare providers of patient discharge through coordination notes.
  • Initiate requests for medical records and hospital inpatient charts, and process provider notifications relevant to patient care.
  • Facilitate coordination among the interdisciplinary team (IDT) for a holistic approach to patient care.
  • Act as a central point of contact for the hospice team, patients, and families, ensuring clear and efficient communication.
  • Handle incoming telephone calls, take messages, and transfer calls to the appropriate personnel.
  • Manage the phone system and switch to the secondary system when necessary.
  • Assist the Hospice Director of Clinical Services (DCS) with pre- and post-IDT activities, including preparing agendas and updating plans of care (UPOCs).
  • Send UPOCs to attending physicians via mail or fax.
  • Prepare binders for patients in facilities.
  • Perform other duties as assigned.
