About The Position

The Hospice Spiritual Coordinator plays a vital role in providing compassionate spiritual care and support to patients and their families during end-of-life journeys. This position ensures that the spiritual needs of hospice patients are met with sensitivity, respect, and professionalism, regardless of their faith or belief system. The coordinator collaborates closely with interdisciplinary hospice teams to integrate spiritual care into the overall treatment plan, enhancing the holistic well-being of patients. They also serve as a resource and advocate for spiritual matters within the hospice organization, facilitating meaningful conversations and rituals that honor patients' values and traditions. Ultimately, this role contributes to creating a supportive environment that promotes peace, dignity, and comfort for patients and their loved ones during a profoundly challenging time.

Requirements

• Bachelor’s degree in Theology, Divinity, Pastoral Care, Counseling, or a related field.
• Certification or ordination as a chaplain, spiritual care provider, or equivalent recognized credential.
• Minimum of two years’ experience providing spiritual care, preferably in a hospice, palliative care, or healthcare setting.
• Strong understanding of diverse religious, spiritual, and cultural traditions and practices.
• Excellent interpersonal and communication skills with the ability to provide empathetic support to patients and families.
• Strong interpersonal and active listening skills daily to build trust and provide meaningful spiritual support to patients and families facing end-of-life challenges.
• Knowledge of diverse religious and cultural traditions enables them to tailor spiritual care interventions that honor individual beliefs and values.
• Effective communication skills are essential for collaborating with interdisciplinary teams and educating staff on spiritual care best practices.
• Organizational skills are employed to manage documentation, coordinate spiritual services, and facilitate rituals or ceremonies.
• Empathy and emotional resilience are critical to navigating sensitive conversations and providing comfort during difficult times.

Nice To Haves

• Master’s degree in Divinity, Theology, Pastoral Counseling, or related discipline.
• Board certification as a Hospice and Palliative Care Chaplain (CHPC) or equivalent.
• Experience working within an interdisciplinary hospice or palliative care team.
• Training in grief counseling, trauma-informed care, or bereavement support.
• Familiarity with electronic medical records (EMR) systems for documentation purposes.

Responsibilities

• Conduct comprehensive spiritual assessments for hospice patients to identify their unique spiritual needs and preferences.
• Provide individualized spiritual counseling, guidance, and support to patients and their families, respecting diverse religious and cultural backgrounds.
• Collaborate with medical, nursing, social work, and bereavement teams to integrate spiritual care into the overall hospice care plan.
• Facilitate spiritual rituals, ceremonies, or practices as requested by patients or families, including end-of-life rites and memorial services.
• Offer education and training to hospice staff on spiritual care principles and cultural competency to enhance interdisciplinary understanding.
• Serve as a liaison between patients, families, and community faith leaders or spiritual resources when appropriate.
• Document spiritual care interventions and patient interactions accurately in compliance with hospice policies and regulatory standards.
• Participate in quality improvement initiatives to continually enhance the spiritual care services provided by the hospice program.
