About the Role As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end-of-life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time. Position likely to grow to as more hours are needed.

Requirements

  • Masters Degree in Social Work from a CSWE accredited program.
  • Licensed as a Social Worker through the Massachusetts Board of Social Worker.
  • Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred.
  • Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.
  • Strong communication, counseling, and interpersonal skills
  • Ability to work independently while remaining an engaged member of a multidisciplinary team
  • Compassionate, empathetic, and patient-centered approach to care

Nice To Haves

  • at least one in hospice or homecare preferred

Responsibilities

  • Conduct psychosocial assessments of patients and families
  • Provide emotional support, counseling, bereavement and crisis intervention as needed
  • Assist patients and caregivers with advance care planning and end-of-life decision-making
  • Connect families with community resources and supportive services
  • Facilitate discussions regarding patient goals, family dynamics, coping, and bereavement needs
  • Collaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary team
  • Advocate for patient and family needs while promoting dignity, comfort, and quality of life
  • Complete timely and accurate documentation in accordance with hospice standards
