About The Position

Paradigm Health is hiring a full-time Hospice Social Worker to provide psychosocial support and social services to terminally ill patients and their families. The role serves counties surrounding the nearest office and works in coordination with patients, families, hospice staff, and community services. The position is salaried, with mileage reimbursement, and includes participation in a weekend on-call rotation.

Requirements

A Master of Social Work (MSW) degree is required at a minimum.
A current Licensed Social Worker credential in Indiana (LSW or LCSW) is required.
At least one year of healthcare experience is required.
A current driver's license is required.
Reliable transportation and insurance are required.

Nice To Haves

One year of hospice experience is preferred, but not required.

Responsibilities

Assess the psychosocial and spiritual needs of patients and families.
Provide social service activities for terminally ill patients and their families, including planning, treatment, and consultation with other staff.
Assist with funeral planning.
Assist in developing and implementing the patient/family Plan of Care.
Complete the Initial Psychosocial Assessment within five calendar days of patient admission.
Provide counseling to patients, families, and hospice staff as needed.
Participate in bereavement follow-up services as required.
Maintain complete documentation of activities and services provided to patients and families.
Assist in developing support groups and other special programs as necessary.
Facilitate respite and long-term care placement.
Provide referrals to community services as appropriate.
Work as a team member.
Participate in the weekend on-call rotation.
Perform other duties as assigned.
