About The Position

Serving communities across Washington, our team of dedicated professionals provides comfort, dignity, and compassionate support to patients and families during one of life’s most meaningful journeys. We are seeking a compassionate Hospice Licensed Clinical Social Worker to provide hospice care throughout King County. Care is delivered across diverse settings, including private homes, adult family homes, assisted living communities, and skilled nursing facilities. In this role, you will coordinate and manage end-of-life care by addressing patient and family needs, ensuring each individual receives high-quality, dignified care that aligns with their goals, values, and wishes.

Requirements

  • Master’s degree in social work (MSW) from an accredited program
  • Current Washington State LSWAIC or LICSW license required
  • Active Basic Life Support (BLS) certification
  • Valid driver’s license and reliable transportation
  • Excellent interpersonal and communication skills
  • Compassionate, patient-centered approach
  • Ability to work collaboratively in an interdisciplinary environment
  • Strong organizational and time management skills
  • Proficiency in electronic health records (EHR) and standard healthcare technologies

Nice To Haves

  • Experience in hospice, palliative care, or related healthcare setting is strongly preferred

Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient and family needs
  • Provide supportive counseling addressing emotional, psychological, and grief-related concerns
  • Connect patients and families with community resources and support services
  • Collaborate with the interdisciplinary team to develop individualized psychosocial care plans
  • Educate patients and caregivers about hospice services, advance care planning, and coping strategies
  • Offer crisis intervention and emotional support during urgent situations
  • Maintain accurate, timely documentation in accordance with state, federal, and agency standards
  • Advocate for patient rights, preferences, and culturally sensitive care
  • Participate in care team meetings and contribute to coordinated care strategies
  • Stay informed on best practices in hospice care through ongoing education
