About The Position
Good Shepherd Community Care, a community-based, mission-driven hospice and palliative care organization, is seeking a Hospice Social Worker to join its expanding counseling services team. This role is integral to an interdisciplinary hospice team, providing comprehensive psychosocial care to patients and families at the end of life. The position requires travel to patient homes and residential facilities throughout Greater Boston and parts of Metrowest. Requirements
- MSW required
- LICSW preferred
- Sensitivity and understanding of end-of-life issues
- Psychosocial assessment and treatment expertise
- Excellent organizational and time management skills
- Clear communication
- Ability to contribute as part of a collaborative team
Nice To Haves
- Experience/skills in utilizing spirituality/religion practices in end-of-life work preferred
Responsibilities
- Provide comprehensive psychosocial care to patients and families at the end of life as part of an interdisciplinary hospice team.
- Make home visits to patients.
- Facilitate referrals and admissions to services.
- Provide counseling support during the bereavement period.
