Requirements

Master’s degree in Social Work from an accredited program.
Minimum of one (1) year of healthcare experience; hospice experience preferred.
Strong verbal and written communication and organizational skills.
A passion for providing compassionate, patient-centered care.
A valid driver’s license and reliable, insured transportation.

Responsibilities

Provide compassionate care to hospice patients and their families, offering psychosocial support during the end-of-life journey.
Conduct comprehensive psychosocial assessments and develop care plans that address emotional, family, and cultural factors affecting the patient’s well-being.
Collaborate with an interdisciplinary team to ensure the delivery of comprehensive, holistic care that meets the patient’s needs.
Offer counseling, crisis intervention, and support to patients and families facing the emotional and practical challenges of terminal illness.
Educate patients and families on advanced directives and assist in planning for end-of-life decisions.
Serve as a liaison between patients, families, and community agencies, helping to connect them with valuable resources and services.
