About The Position
A local hospice agency is in need of a temporary hospice social worker for 2-3 months primarily in St. Charles and west of St. Charles. This position is part-time to cover a caseload of 5-10 patients. Hours are Monday-Friday daytime, visits may be scheduled anytime the social worker and patient/family wish.
Requirements
- MSW required.
- Must be comfortable making home visits.
- Current driver's license and reliable transportation.
Nice To Haves
- Home health or hospice experience preferred.
Responsibilities
- Complete psychosocial, financial, environmental, and community needs assessments and documentation in a timely manner.
- Assist with advance directives, funeral planning, and grief and end-of-life counseling.
- Attend IDT meetings and participate in care planning as needed.
