About The Position

National Church Residences Home Health division, located in Southern Ohio, is a Home Health & Hospice agency seeking a Social Worker to join their team. This role is full-time, Monday-Friday, 8 am-4:30 pm, with an on-call rotation. The agency is owned and operated by National Church Residences, a leading provider of senior housing and healthcare services. The Social Worker will contribute to the vision of advancing better living and care for seniors.

Requirements

Bachelor’s or Master's Degree in social work from a Council on Social Work Education accredited school.
LSW in good standing.
One year of social work experience in a health care setting.

Nice To Haves

Minimum of two (2) years in varied multi-disciplinary settings in the health care field.
Experience working with clients and family counseling.

Responsibilities

Provide social work services within a hospice setting.
Conduct client and family counseling.
Participate in an on-call rotation.
