About The Position

Care Initiatives Hospice is seeking a compassionate Social Worker to provide comprehensive care to patients during their final stages. The role involves meeting patients and families in their preferred locations (home, long-term care, assisted living, hospital) to offer personalized care. The hospice embraces a holistic approach, addressing physical, emotional, and spiritual needs, and collaborates with family members and caregivers to honor patient wishes. The Social Worker will provide care with compassion, dignity, and respect during this significant life phase, aiming to make a difference in the lives of patients and their families.

Requirements

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
• Current, valid social work license (LMSW)
• Current driver's license
• Automobile liability insurance
• Ability to travel between multiple locations within your region

Nice To Haves

• Hospice or palliative care experience
• Two (2) years of verifiable experience working as a Social Worker

Responsibilities

• Assess patients’ psychosocial needs and complete social histories
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support patients and families with adjustment, rights, and orientation
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support patient, family, and community engagement activities
• Promote facility programs and services to referral partners
