About The Position

LifeChoice Hospice & Palliative Care is seeking an experienced Hospice Social Worker to join its interdisciplinary team. The Social Worker supports hospice patients and families with emotional, social, financial, and practical needs throughout end-of-life care. The role works closely with nurses, physicians, chaplains, CNAs, families, facilities, and other members of the hospice care team. The position manages an assigned caseload and completes field visits as needed.

LifeChoice Hospice & Palliative Care is a nurse-led hospice organization serving patients and families throughout the Chicagoland area. The organization focuses on compassionate, responsive care and strong teamwork across disciplines. The role is suited to a Social Worker who understands hospice, communicates well, stays organized, and is comfortable supporting patients and families during important moments.

Requirements

• Master's degree in Social Work.
• Current Illinois social work license appropriate for the position.
• Previous hospice social work experience; hospice experience is required.
• Strong knowledge of hospice and end-of-life care.
• Strong communication and documentation skills.
• Ability to work independently and as part of an interdisciplinary team.
• Reliable transportation for patient and facility visits.

Responsibilities

• Complete psychosocial assessments and ongoing social work evaluations.
• Provide emotional support, counseling, education, and resource coordination.
• Assist patients and families with end-of-life planning and changing care needs.
• Identify social, financial, environmental, and caregiver concerns.
• Connect patients and families with appropriate community resources.
• Participate in interdisciplinary team meetings and care planning.
• Maintain accurate and timely clinical documentation.
• Communicate closely with patients, families, facilities, and the hospice team.
• Assist with advance care planning within the social work scope of practice.
• Manage an assigned caseload and complete field visits as needed.
