About The Position

Suncoast Hospice of Hillsborough, part of Empath Health, is seeking a Social Worker to support patients and families during the end-of-life journey in North Hillsborough County. The role provides psychosocial support, care coordination, and resource assistance to promote comfort and dignity. The position involves assessing patient and family needs, providing counseling and crisis support, coordinating care with an interdisciplinary team, helping families access resources and plan for care, maintaining records, and educating the community about hospice care.

Requirements

Minimum one year of social work experience in a healthcare setting; internships may qualify.
Must hold, or be eligible for within 90 days of hire, one of the following Florida licenses: LCSW or RCSWI; LMHC or RMHCI; or LMFT or RMFTI.
For field-based positions, reliable transportation is required to perform tasks and responsibilities in a timely and appropriate fashion.
Must provide proof of valid automobile insurance for field-based positions.

Responsibilities

Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
Provide individual and family counseling, crisis intervention, and bereavement support.
Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
Assist with financial resources, Medicare/Medicaid, and long-term care planning.
Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
Support patients and families with advance directives and memorial planning.
Educate the community on hospice care.
