About The Position

Ovation Hospice of Denver is seeking a part-time Medical Social Worker to provide psychosocial assessment, counseling, and care coordination for hospice patients and their families across the Denver metro area. The role helps patients and families navigate the emotional, practical, and financial challenges of a terminal diagnosis. This is a field-based position serving patients in private residences, assisted living communities, and skilled nursing facilities. The schedule is flexible, and the social worker works independently while managing a caseload and collaborating with the interdisciplinary team.

Requirements

Master’s degree in Social Work (MSW).
Current Colorado license (LSW or LCSW).
Strong assessment, crisis intervention, and communication skills.
Comfort working independently in the field and managing a caseload.
Valid Colorado driver’s license, reliable transportation, and current auto insurance.
Proficiency with electronic medical records.

Nice To Haves

LCSW preferred.
Experience in hospice, palliative care, home health, or medical social work preferred.

Responsibilities

Complete initial and ongoing psychosocial assessments within required timeframes.
Provide emotional support and short-term counseling to patients, families, and caregivers.
Help families with advance directives, MOST forms, funeral planning, and end-of-life decisions.
Connect families with community resources, including Medicaid, VA benefits, caregiver support, and placement options.
Participate in Interdisciplinary Group (IDG) meetings and contribute to individualized plans of care.
Identify caregiver stress, safety concerns, and risk factors, and escalate them appropriately.
Coordinate with the bereavement team to support families after a death.
Document visits accurately and on time in the EMR in accordance with Medicare and CHAP standards.
