About The Position

This position involves planning, coordinating, and implementing Social Services for patients of all ages, their families, and other significant individuals at the Medical Center. The role requires anticipating and addressing patients' specific psychosocial needs, financial concerns, equipment needs, educational requirements, and community resource access from admission through transfer to the next appropriate level of care. Responsibilities also include assisting with discharge planning, grief counseling, financial applications, and connecting patients to community resources as needed, all with the goal of enhancing patient care and contributing to the Medical Center's mission.

Requirements

A Master's degree in social work from an accredited social work (MSW) program
Current State Social Work Licensure
For Duluth Hospice: Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency.
For Duluth Hospice: Proof of current motor vehicle insurance.

Responsibilities

Plans, coordinates, and implements Social Services for patients of all ages, patients’ families, and other person significant to patients at the Medical Center.
Anticipates and attends to patient’s specific psychosocial needs, financial needs/concerns, equipment needs, educational needs, and community resources from admission through transfer to the next appropriate level of care.
Assists with discharge planning, grief counsel, financial application, and access to community resources as needed.
Enhances patient care, thus contributing to the goals and mission of the Medical Center.
