About The Position

The Medical Social Worker provides psychosocial assessments and counseling services to hospice patients and their families under the direction of the attending physician. The role collaborates with the Interdisciplinary Group/Team (IDG/T) to coordinate care and ensure patients receive holistic, compassionate support tailored to their needs.

Requirements

• Master’s degree in social work (MSW) from an accredited school of social work.
• Current state licensure as a social worker (LCSW, LMSW, or equivalent).
• At least two years of experience in a healthcare setting.
• Strong working knowledge of community resources and support services.
• Excellent interpersonal and communication skills.
• Ability to use computers for documentation and communication.
• Compassionate, empathetic, and professional demeanor.
• Reliable transportation and valid auto liability insurance.

Nice To Haves

• Experience in hospice or palliative care is preferred.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, social, and environmental factors affecting patients and families.
• Provide emotional support and counseling to patients and families coping with end-of-life issues.
• Coordinate care with the IDG/T to ensure all aspects of patient care are addressed.
• Assist families in understanding and managing grief and loss.
• Help families access community resources and services.
• Document social work services in accordance with hospice policies and regulatory requirements.
