Hospice Social Worker - 1.0 FTE

About The Position

We’re seeking a dedicated Hospice Medical Social Worker (MSW) to join our interdisciplinary hospice team, including physicians, nurses, therapists, chaplains, and volunteers. You’ll complete comprehensive bio‑psychosocial assessments, support care planning, and serve as a trusted advocate for patients and families throughout their hospice journey. This role is ideal for someone who brings empathy, strong clinical judgment, and a deep commitment to supporting patients at end of life.

Requirements

  • Master of Social Work (MSW) from a CSWE‑accredited program (required)
  • Minimum two years of social work experience
  • Knowledge of acute, chronic, and terminal illness
  • Strong interpersonal communication, assessment, and critical decision‑making skills
  • Ability to work independently and collaboratively within a care team
  • Knowledge of community resources and support systems

Nice To Haves

  • Experience in hospice, home health, or community‑based programs preferred
  • Optional: LCSW licensure in the State of Montana

Responsibilities

  • Conduct comprehensive bio‑psychosocial assessments, including support systems, living arrangements, financial resources, mental health history, and care goals
  • Provide emotional support, counseling, and education to patients and families coping with terminal illness
  • Participate in daily interdisciplinary huddles and ongoing care planning
  • Assist with advance directives, POLST forms, medical power of attorney, and guardianship processes
  • Support families through grief, loss, and bereavement, connecting them with appropriate resources
  • Help patients navigate insurance, disability applications, and financial assistance programs (VA, SSI, Medicare, Medicaid, private insurance)
  • Coordinate community resources such as transportation, meals, respite care, and long‑term care placement
  • Serve as a liaison between patients, families, and healthcare providers to ensure understanding of medical information
  • Assist with transitions to nursing homes, assisted living, or outside hospice programs when needed
  • Collaborate with hospice chaplains to arrange faith‑based support
  • Identify barriers to care and help patients set goals to support treatment compliance and well-being
