About The Position

Provide medical social services to the patient and/or family according to the Plan of Care. Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient's care. Participate in IDG meetings and activities and the development of the patient plan of care. Observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and response to treatment. Maintain and submit written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes. Confirm weekly, the scheduling of visits with the patient's case managers to facilitate coordination of other staff visits. Assess the patient's and family member's ability to cope with the patient's passing. Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems. All other duties as assigned

Requirements

  • Licensed Social Worker in state of practice (LSW)
  • Current Driver's License, vehicle insurance, and access to dependable transportation.

Nice To Haves

  • Master's Degree in social work preferred (MSW)
  • One year of social work experience in a healthcare setting is preferred
