About The Position
A respected community hospital in Arkansas, just outside of Branson, Missouri, is seeking a Hospice Social Worker to join their Home Health & Hospice team. This is a full-time, permanent, direct-hire opportunity. The role involves being a core member of the Hospice team, providing support to patients and families facing terminal illness, and managing patient transitions and care coordination.
Requirements
- Must have an Arkansas LCSW or LMSW or be eligible to obtain one.
- Must have or be eligible to obtain BLS Certification.
Responsibilities
- Provide support to patient's and family's emotional, social, and economic needs relating to the impact of a terminal illness.
- Plan the transition and transportation of patients from hospitals to a home or another facility.
- Arrange services for patients.
- Collaborate with doctors, nurses, and insurers.
- Plan and lead interdisciplinary meetings.
- Facilitate referrals for community resources.
- Review critical access admissions and monitor patients with regularly scheduled admission visits.
