We are seeking a dedicated Part-Time Hospice Social Worker (MSW) to provide individualized hospice support services at the Kate B. Reynolds Hospice Home. Requirements

• Master’s Degree in Social Work (MSW) required
• Knowledge of community social, financial, and healthcare resources
• Strong written and verbal communication skills
• Ability to work independently and within an interdisciplinary team
• Commitment to hospice philosophy and compassionate care
• Excellent organizational and prioritization skills

Responsibilities

• Provide individualized hospice services to patients and families
• Assess emotional, social, and financial needs
• Develop psychosocial goals within patient care plans
• Support patients and families in accessing community resources
• Collaborate closely with the interdisciplinary team (IDT)
• Attend and contribute to IDT meetings
• Assist with discharge planning and transitions of care
• Educate families regarding Hospice Medicare/Medicaid benefits
• Complete required clinical documentation accurately and timely
• Support orientation of new staff, interns, and volunteers
• Participate in Social Work on-call duties
