Requirements

Master's Degree in Social Work (MSW)
Valid Florida driver's license
Reliable transportation and current auto insurance

Nice To Haves

Two years of healthcare, hospice, home health, oncology, palliative care, or related experience
Florida Licensed Clinical Social Worker (LCSW) or license-eligible
Knowledge of grief and bereavement counseling
Experience working with interdisciplinary healthcare teams
Strong communication, organizational, and counseling skills

Responsibilities

Complete comprehensive psychosocial assessments
Provide emotional support, counseling, and crisis intervention for patients and families
Assist with advance care planning and community resource referrals
Help families navigate Medicaid applications, financial resources, and long-term care planning
Collaborate closely with physicians, nurses, chaplains, volunteers, and other members of the hospice team
Participate in interdisciplinary care conferences
Advocate for patients and families while honoring their goals and wishes
Provide services wherever patients call home—including private residences, assisted living facilities, nursing homes, hospitals, and inpatient hospice care
