About The Position
Sentara Health is seeking to hire a qualified individual to join our team as a Hospice Social Worker - MSW. The Hospice Social Worker - MSW performs a variety of casework and provides social work services to patients, families, and significant others. This role involves developing and monitoring multidisciplinary collaboration of services to patients where appropriate. The Social Worker will assist adult patients and their families with personal and environmental difficulties associated with their medical condition, up to and including the time of terminal illness. Skills in interviewing techniques and conducting group sessions are essential.
Requirements
- Master's Level Degree in Social Work (Required)
- BLS (Required within 90 days of hire)
- Valid Driver's License (Required)
- Hospice (previous experience required)
- Home Health (previous experience required)
- Social Work (previous experience required)
- Group Social/Rec Program (previous experience required)
Responsibilities
- Performs a variety of casework and provides social work services to patients, families, and significant others.
