Requirements

  • Leads with empathy, presence, and strong listening skills
  • Is comfortable having meaningful and sometimes difficult conversations
  • Advocates for patients and families
  • Works well independently while collaborating with a team
  • Is organized and follows through
  • Believes social work is about both support and empowerment
  • Master’s degree in Social Work (MSW)
  • Licensed Clinical Social Worker (LCSW) in Oregon
  • Strong communication and organizational skills
  • Valid driver’s license and reliable transportation

Nice To Haves

  • 2+ years healthcare experience preferred (hospice or home health a plus)
  • Current BLS certification

Responsibilities

  • Provide psychosocial assessments and ongoing support
  • Help patients and families navigate difficult decisions
  • Educate on advanced directives and end-of-life planning
  • Connect families with community resources
  • Support caregivers through emotional and practical challenges
  • Collaborate with the interdisciplinary hospice team
  • Conducting psychosocial assessments in patients’ homes
  • Developing and implementing care plans
  • Supporting patients and families with emotional and practical needs
  • Educating on advance directives and care planning
  • Coordinating with community resources and services
  • Participating in interdisciplinary team meetings
  • Providing support across hospice (with occasional home health collaboration)
