About The Position
Make a meaningful impact by supporting patients and families through every stage of the end-of-life journey. We are seeking a compassionate and experienced Hospice Social Worker to provide psychosocial support, counseling, care coordination, and community resource assistance for hospice patients and their loved ones.
Requirements
- Master's Degree in Social Work from an accredited program.
- Current Social Worker license required.
- Minimum 1 year of hospice experience required.
- Strong knowledge of end-of-life care, grief support, and community resources.
- Valid Driver’s License and reliable transportation with current auto insurance.
- Current AHA CPR Certification.

- HomeCare HomeBase experience preferred.
Responsibilities
- Complete comprehensive psychosocial assessments within 5 days of hospice admission.
- Identify and address emotional, social, financial, environmental, and community resource needs.
- Provide counseling and support related to end-of-life issues, grief, loss, and bereavement.
- Assist with advance directives, funeral planning, discharge planning, and family counseling.
- Collaborate with the interdisciplinary team to develop and coordinate individualized patient care plans.
- Participate in IDG meetings and maintain accurate clinical documentation.
- Advocate for patients and families while promoting dignity, respect, and quality of life.
