About The Position

Care Initiatives Hospice is seeking a Social Worker serving Iowa Falls, Hampton, Eldora, and surrounding areas. The role provides compassionate, holistic care to patients with life-limiting illnesses and their families in settings where patients are most comfortable, including homes, long-term care communities, assisted living residences, and hospitals. The Social Worker works collaboratively with family members, caregivers, and the care team to address patients’ physical, emotional, and spiritual needs and to honor their wishes with dignity and respect.

Responsibilities include assessing psychosocial needs, providing counseling and emotional support, documenting care, coordinating referrals, and connecting patients and families with community resources. The Social Worker also supports engagement activities and promotes facility programs and services to referral partners.

Requirements

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.
• Current, valid social work license (LMSW).
• Current driver's license and automobile liability insurance.
• Ability to travel between multiple locations within the region.

Nice To Haves

• Hospice or palliative care experience.
• Two years of verifiable experience working as a Social Worker.

Responsibilities

• Assess patients’ psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support patients and families with adjustment, rights, and orientation.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support patient, family, and community engagement activities.
• Promote facility programs and services to referral partners.
