About The Position

The Social Service Weekend Coordinator, contracted or employed through Suncrest Hospice, is responsible for implementing standards of care for medical social work services and adhering to all conditions in the Service or Employment Agreement. The primary responsibility is coordinating PRN staff and visits, with other duties as applicable. This role involves working closely with social workers, chaplains, and nursing leadership to ensure patient needs are met on weekends. It requires timely responses, effective clinical assessment, and the ability to work independently. The position is remote.

Requirements

  • A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education
  • Demonstrates good verbal and written communication skills.
  • Excellent organizational skills, ability to prioritize, work independently.
  • Strong technical skills for EMR documentation, reporting and auditing.
  • Must be a licensed driver with an automobile in good working condition, that is insured in accordance with state or organization requirements and is in good working order.
  • Understands hospice philosophy, and issues of death/dying.
  • Experience in a hospice care or home health preferred.

Responsibilities

  • Assess and prioritize patient and family needs and schedule PRN and Part-time Social Workers and Chaplains for patient visits from Friday evening-Sunday.
  • Collaborate with the weekend nursing director and team to triage urgent psychosocial patient issues.
  • Speak with patients, families, senior living facilities, and community agencies to address time-sensitive needs of patients.
  • Respond to urgent patient and family needs, including but not limited to advance care planning, assisting with funeral arrangements, urgent respite placement in a skilled nursing facility, and assisting with finding caregivers.
  • Conduct evaluations of family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse, and plan interventions based on evaluation findings.
  • Counsel patients and families/caregivers as needed in relation to stress and other identified coping difficulties.
  • Provide crisis intervention when necessary.
  • Conduct social work visits when indicated when PRN or On-Call staff are unavailable.
  • Complete charting in the EHR in a timely manner and maintain clinical records on all patients referred to social services.
  • Provide information and referrals for patients and families/caregivers regarding practical needs and community agencies.
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Assist Interdisciplinary care team members in understanding significant psychosocial and emotional factors related to medical condition and end-of-life concerns.
  • Other duties as delegated by the supervisor.
