About The Position

Hospice of Washington County is seeking a Hospice RN for Doey's House on the Night Shift. This role involves providing quality nursing care to patients with life-limiting illnesses, focusing on maintaining the highest level of care in conjunction with the primary physician. The RN will participate as a member of the interdisciplinary team to develop and implement individualized care plans, consult with physicians, and evaluate patient/family responses to interventions. Education and family teaching regarding infection control and maintaining quality care are key components. The position also includes documenting patient care, participating in Quality Improvement programs, and providing direction to support staff and volunteers. The RN will also complete daily acuity assessments and attend to patients and families at the time of death. GBMC HealthCare and its affiliates are Equal Opportunity employers. Requirements

• ADN and/or BSN from an accredited institution.
• Two years of nursing experience, or equivalent combination or education and experience.
• Knowledgeable of universal precautions for infection control and employs safe practices for self protection
• Strong time management skills
• Excellent interpersonal and communication skills
• Ability to be patient and family focused
• Skill in the use of EMR (Electronic Medical Record) and/or ability to learn
• Current MD registered nursing license.
• Ability to stand and walk almost constantly up to ninety 90% of work time and lifting and positioning patients up to fifty 50% of work time.
• Ability to concentrate and pay close attention to detail and often in stressful situations.
• Maintain current nursing license

Responsibilities

• Provides quality nursing care to patients with life-limiting illness to maintain highest level of care for the patient in conjunction with the primary physician by initiating and maintaining the nursing process.
• Participates as a member of the interdisciplinary team to develop and implement a patient directed individualized plan of care.
• Consults with attending physician when necessary.
• Evaluates patient/family response to intervention and modifies care plan appropriately.
• Provides patient and family education to achieve skills necessary to maintain quality care, promote patient/family independence and to facilitate the patient living fully until death.
• Teach patient/family infection control protocols.
• Documents on appropriate chart and forms in an accurate and timely manner to provide a written record of inpatient course.
• Participates in Quality Improvement Program to maintain or improve the quality of patient care.
• Provides direction and supervision to Patient Care Partners, Unit Receptionists and volunteers to maintain environment.
• Accurately completes daily acuity assessment for assigned patient.
• Attends to patient and family at the time of patient’s death.
