About The Position

Hillcrest Health & Living is looking for a compassionate, clinically strong Hospice RN to join our Hillcrest Hospice team in Omaha, NE. Requirements

  • At least three years of nursing experience.
  • Strong clinical assessment, symptom-management, and care-coordination skills.
  • Excellent written and verbal communication skills.
  • Strong problem-solving and decision-making abilities.
  • Ability to build rapport with patients and families during sensitive and emotional situations.
  • Ability to work independently while staying connected to the interdisciplinary team.
  • Strong attention to detail with documentation, care planning, and follow-through.
  • A calm, flexible, and professional approach in changing or urgent situations.
  • Willingness to travel to multiple work locations.
  • Graduate of an accredited nursing program required.
  • Active Registered Nurse license in Nebraska or compact state required.
  • Valid driver’s license required.
  • Reliable transportation and current automobile insurance required.
  • Basic Life Support certification through the American Heart Association required.
  • Must be able to speak, read, and write English.
  • Must be able to independently lift up to 35 pounds.
  • Must be able to perform repetitive sitting, bending, standing, walking, and movement as needed.
  • Must be flexible, calm, and professional in emotionally sensitive or stressful situations.
  • Must be able to work in a fast-paced environment with strong attention to detail.
  • Must be able to utilize technology, clinical systems, and resources efficiently and effectively.
  • Must be able to travel to multiple work locations.
  • Must maintain current driver’s license and auto insurance coverage within required limits.
  • Possible exposure to communicable diseases and bodily fluids.
  • On-call responsibilities may be required.

Nice To Haves

  • Hospice, home care, palliative care, long-term care, hospital, or similar experience preferred.
  • Bachelor’s degree preferred.

Responsibilities

  • Provide nursing visits and assess patients’ physical, functional, psychosocial, spiritual, cultural, environmental, and nutritional needs.
  • Manage patient care with focus on bedside care, symptom management, crisis intervention, and family support.
  • Coordinate and integrate services provided by all hospice disciplines.
  • Teach patients and families about disease progression, self-care techniques, end-of-life care, and ethical concerns.
  • Communicate professionally with physicians, patients, families, caregivers, and team members to support the plan of care.
  • Notify patients and caregivers of schedules and schedule changes as needed.
  • Communicate changes in patient condition or care needs to leadership and appropriate team members.
  • Provide nursing services in accordance with each patient’s plan of care.
  • Prepare accurate clinical notes, progress notes, assessments, and care plan updates.
  • Initiate, document, and revise hospice aide care plans based on patient condition.
  • Prepare documentation for interdisciplinary team meetings and communicate recommended interventions.
  • Participate in team conferences, service line meetings, QAPI activities, committees, and ongoing education.
  • Provide consultation, education, and support to internal and external stakeholders.
  • Participate in on-call and backup call rotations as needed to support patient care.
