About The Position

Mo Cara Hospice Care is dedicated to providing compassionate, patient-centered care to individuals at the end of their life journey. We offer comfort, dignity, and peace to both residents and their families. We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our interdisciplinary team and provide holistic end-of-life care to our patients and their families. The ideal candidate will be committed to our mission of ensuring patient comfort, dignity, and quality of life through expert symptom management, emotional support, and effective communication. This role involves developing and implementing personalized care plans in collaboration with physicians, social workers, spiritual counselors, and other team members. This person would be asked to travel to Rockford, McHenry, Hoffman Estates and Huntley areas.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Illinois.

Responsibilities

  • Coordinate all aspects of patient care, serving as the primary point of contact for patients and families.
  • Conduct thorough initial and ongoing nursing assessments to evaluate patients' physical, emotional, and psychosocial needs.
  • Develop and update individualized patient care plans in partnership with the interdisciplinary group (IDG).
  • Administer medications and treatments to effectively manage symptoms and control pain, ensuring maximum comfort for the patient.
  • Educate patients, families, and caregivers on the disease process, symptom management techniques, and end-of-life care.
  • Provide empathetic emotional and spiritual support to patients and their loved ones throughout the end-of-life journey.
  • Supervise Licensed Practical Nurses (LPNs) and certified nursing assistants (CNAs) to ensure quality and compliant care.
  • Maintain timely and accurate patient records, including clinical notes and care updates, in compliance with all regulations.
  • Respond to patient and family needs and provide crisis care during a scheduled on-call rotation.
  • Participate in IDG meetings to discuss patient status and update care plans.
