About The Position

Constellation Health Services is seeking a compassionate and skilled per diem/PRN Registered Nurse to care for hospice patients and their families in their homes, assisted living facilities, and skilled nursing facilities in Bangor, Maine. The role includes providing nursing care in patients’ home settings and coordinating care with patients, families, caregivers, and other disciplines. Rates are $70 per visit Monday through Friday and $80 per visit on Saturdays and Sundays; start-of-care visits are $150 Monday through Friday and $165 on Saturdays and Sundays. Travel is up to 25%. Weekend and holiday coverage may be needed. The position uses a computer-based documentation system, and clinical documentation, time records, and other required information must be submitted within the required time frame.

Requirements

• Graduate of an accredited RN program.
• Registered Nurse license in the state of employment.
• At least 1–2 years of RN experience.
• Basic knowledge of the iOS operating system.

Nice To Haves

• Previous hospice care experience is strongly desired.

Responsibilities

• Provide skilled nursing care, including administering medications and treatments prescribed in the physician’s plan of care.
• Perform comprehensive patient assessments and coordinate the plan of care with relevant disciplines, family members, and primary caregivers.
• Observe and evaluate patients’ physical and emotional status and report changes to the nursing supervisor and attending physician.
• Participate in weekly Interdisciplinary Team Meetings and provide updates on patients’ physical, emotional, psychosocial, and spiritual status.
• Maintain and process documentation in compliance with agency standards.
• Use the computer-based documentation system and submit clinical documentation, time records, and other required information within the required time frame.
• Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluations.
• Provide weekend and holiday coverage as needed.
