About The Position

Requirements

  • Must be available to work full-time in the Santa Clara County area on the PM shift.
  • Current CA Registered Nurse (RN) license
  • Current CPR card for Healthcare Providers
  • Minimum one (1) year experience as an RN preferably in hospice or palliative care
  • Minimum one (1) year of Hospice/home health experience preferred
  • Must be computer proficient in typing and various programs, including a background in EMR preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Understands the philosophy of Hospice concepts.
  • Experience with the terminally ill patient.
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
  • Self-directed and able to work with minimal supervision.

Nice To Haves

  • Minimum one (1) year experience as an RN preferably in hospice or palliative care
  • Minimum one (1) year of Hospice/home health experience preferred
  • Background in EMR preferred.

Responsibilities

  • Answering all calls, meeting patient needs, and making visits.
  • Providing support to patients and family members following the plan of care.
  • Answering calls for clinical issues.
  • Reviewing clinical referral information and responding to requests and inquiries.
  • Collaborating with the Director of Patient Care regarding the eligibility and appropriateness of clients for hospice services.
  • Performing ongoing comprehensive assessments of the patient and family to determine hospice needs.
  • Providing a complete physical assessment and history of current and previous illness(es) while being aware of the physical, emotional, and spiritual aspects.
  • Gathering data on social, economic, and cultural factors that may influence health, well-being, and quality of life.
  • Coordinating patient care for assigned caseload and providing direct care to patients, maintaining the highest level of comfort and quality of life.
  • Applying nursing principles and techniques in the treatment of patients.
  • Providing nursing care, emotional support, and education to patients and families.
  • Encouraging patients and family members to participate in the patient's plan of care to meet established goals.
  • Coordinating with hospice physicians for required in-person or virtual face-to-face meetings.
  • Participating in On-Call duties as assigned and defined by the Director of Patient Care.
  • Supervising LVNs and HHAs, ensuring they are following the patient's plan of care.
  • Participating in the on-call and holiday work rotation.
  • Being trained on how to do hospice admissions as a backup.
