Requirements

• Registered Nurse with Valid CT RN license
• One year of RN experience
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Support patients, families, and loved ones through their end-of-life journey.
• Perform comprehensive assessments to establish goals for symptom management and end-of-life care with effective case management strategies.
• Be a member of an active interdisciplinary team that addresses patients’ needs holistically.
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
