About The Position
Providence Hospice and Palliative Care is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a Hospice Registered Nurse (RN), PRN in Homer Glen, IL and surrounding suburbs. We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. We provide exceptional patient centered care to patients and families facing end of life. Requirements
- Active Registered Nurse (RN) License in the state of Illinois.
- At least 1 year of RN experience, preferably in Hospice or Home Health.
- Drivers License in good standing.
Responsibilities
- Provide skilled nursing care.
- Provide case management services.
- Conduct initial assessments of new patients.
- Travel to patients wherever they reside: Home, nursing facility etc.
- Participate in an on-call schedule.
