Hospice RN On-Call (Monday-Thursday)

Paradigm Health is seeking a full-time Hospice RN On-Call to join their team. This role involves providing patient-focused, high-quality care to terminally ill patients and their families. Requirements
- 1 year of RN experience required
- Licensed Registered Nurse in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
Nice To Haves
- One year Hospice experience preferred
Responsibilities
- Performs physical and psychosocial assessment on all hospice patients.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
- Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
- Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
