About The Position

Paradigm Health is an Indiana-owned company seeking a full-time Hospice RN On-Call for Monday-Thursday, 5:00 pm - 8:00 am. The role involves responding to on-call needs within an hour, performing assessments, managing pain, and addressing the holistic needs of terminally ill patients and their families. Requirements

1 year of RN experience required
Licensed Registered Nurse in State of Indiana
Current driver's license
Must have reliable transportation and insurance

Nice To Haves

One year Hospice experience preferred

Responsibilities

Performs physical and psychosocial assessment on all hospice patients.
Ability to adequately assess and reassess pain.
Utilizes appropriate pain management techniques.
Educates the patient and family regarding pain management.
Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
