About The Position

Lifesong Hospice and Palliative Care is seeking a Registered Nurse (RN) with a compassionate 'hospice heart' to join their team. This role involves connecting with clients in their homes to manage symptoms and maintain quality of life, providing peace of mind for patients and their families. The RN will have direct access to a support team and on-call staff. The position is within the Harrisburg and York area, with a schedule of 7 days on and 7 days off, covering hours from 5 pm to 8:30 am.

Requirements

  • Registered nurse RN with current licensure to practice professional nursing in Pennsylvania.
  • Graduate of National League for Nursing accredited school of nursing.
  • Minimum one year of experience.
  • Self-directed and able to work with minimum supervision.
  • Demonstrated ability to prioritize tasks and handle pressure and delicate situations.
  • A state driver’s license in good standing, and maintain an automobile in good working order.
  • High proficiency with computers, Microsoft Office, cellular technology, and EMR required.
  • Able to perform the physical tasks that are needed in this role.

Nice To Haves

  • Experience in the area of palliative care, geriatric care, or hospice nursing preferred.

Responsibilities

  • Completes initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides healthcare instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optimal comfort care.
  • Communicates with community health related persons to coordinate the care plan.
  • Teaches the patient and family/caregiver self-care techniques, as appropriate.
  • Reports the status of schedule and rescheduled assignments, late arrivals, no-shows, and outstanding follow-up appointments to the Team Director/Director of Clinical Operations.
  • Participates in on-call duties as defined by the on-call policy.
