About The Position
Paradigm Health is an Indiana-owned company seeking a full-time Hospice RN On-Call to provide patient-focused, high-quality care. The role involves responding to on-call needs within an hour, performing assessments, managing pain, and addressing the holistic needs of terminally ill patients and their families. Requirements
- One year Hospice experience required
- 1 year of RN experience required
- Licensed Registered Nurse in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
Responsibilities
- Performs physical and psychosocial assessment on all hospice patients.
- Ability to adequately assess and reassess pain.
- Utilizes appropriate pain management techniques.
- Educates the patient and family regarding pain management.
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
- Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
- Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
