About The Position

Requirements

• Graduate of Nursing Program (Associates or Diploma Program)
• Arizona RN License or compact state RN license
• Level One Fingerprint Clearance Card, required within 20 days of hire
• AZ Driver's License
• AZ Food Handlers Card
• Arizona Heart Association or American Red Cross BLS certification
• Must be 21 yoa or older
• One (1) or more years of experience in acute care nursing or hospice
• Able to handle multiple priorities simultaneously
• Possess good oral and written communication skills
• Able to care for patients in hospice inpatient setting, and in home setting as needed
• Able to assist with the movement of an adult patient to 300 lbs
• Able to stand, bend, stoop, kneel, and reach, while working with patients for the entire shift
• Expected duties have routine potential for exposure to blood, body fluids or tissues
• May require travel, in properly insured vehicle, throughout our service area.

Responsibilities

• Coordinate the implementation of a Hospice Plan of Care for each assigned patient.
• Provide nursing care to patients in compliance with nursing standards and regulations, under the direction of the physician and the Arizona State Nurse Practice Act.
• Make continuing assessments of the patient’s total health needs utilizing the nursing process.
• Implement nursing interventions based on assessment data.
• Develop and implement the nursing plan of care and evaluate the patient’s response.
• Educate patients and families about the disease process.
• Help manage and control pain and other symptoms.
• Offer solace to those in distress.
• Support patients and their families at a critical juncture in their lives as part of an interdisciplinary inpatient hospice team.
