About The Position
Join our hospice team as an On-Call Hospice RN and make a meaningful difference in the lives of patients facing life-limiting illnesses. This role provides after-hours nursing support, symptom management, patient assessments, and end-of-life care in patients' homes and care facilities. You will work independently while collaborating with an interdisciplinary team to ensure patients receive exceptional, patient-centered care focused on comfort, dignity, and quality of life.
Requirements
- Current RN license in good standing.
- Current CPR certification.
- Valid driver's license and auto insurance.
- Strong assessment, communication, and critical thinking skills.
Nice To Haves
- Minimum of 2 years of hospice nursing experience preferred.
- HomeCare HomeBase experience preferred.
Responsibilities
- Respond to after-hours patient and family needs from 4 PM to midnight.
- Perform comprehensive patient assessments and evaluate changes in condition.
- Develop, implement, and update individualized plans of care.
- Provide symptom management, medication education, and end-of-life support.
- Educate patients, families, and caregivers on disease progression and care needs.
- Coordinate care with physicians, hospice aides, social workers, chaplains, and other team members.
- Document patient visits and interventions accurately and timely.
- Provide clinical oversight and direction to hospice aides and LVNs/LPNs as needed.
