About The Position

Providence at Home, a division of Providence Life Services, is seeking a Full Time Hospice RN Clinical Supervisor for its Providence Hospice in Homer Glen, IL. This role offers a $5,000 retention bonus. The Hospice RN Clinical Supervisor will lead the Interdisciplinary Team, which includes RNs, Social Workers, and Chaplains, in providing patient-centered care focused on dignity, comfort, and peace for end-of-life patients and their families. Requirements

Active Registered Nurse (RN) License for the state of Illinois.
Hospice or Home health experience is required.
1-2 year(s) of management/supervisor experience.
Knowledge of Hospice Philosophy of Care.
Commitment to Clinical and Documentation excellence.

Responsibilities

Provision of exceptional communication to patients, staff referral sources and physicians.
Education of staff regarding Medicare regulations and Conditions of Participation for Hospice.
Training of new staff on computer software.
Responsible for supervising RN Case Managers and home health aides.
Management of clinical staff while keeping a high level of team spirit and expertise.
Assist with determining and satisfying client requests.
Review referrals and Plans of Care for completeness and accuracy.
On-site assessments of staff for skills proficiency and infection prevention.
