About The Position
Oversees primary care plan of Hospice patients and provides skilled professional nursing care to them, their families and caregivers. Facilitates patient and family education to promote continuity of care and optimal patient/family outcomes for quality of life as defined by the patient/family/caregiver. Documents accurate, complete and relevant clinical notes regarding patient's condition and care given. Maintains documentation a on a timely basis.
Requirements
- Graduate of an accredited registered nursing program
- Minimum of 2 to 4 years as a RN with experience in a hospice or home health agency or other related health program.
- Understanding of Hospice philosophy, principles of death/dying.
- Valid driver's license and reliable transportation that is insured in accordance with Ohio state requirements.
Nice To Haves
- Bachelors degree in Nursing preferred
- Experience with pain and symptom management interventions preferred
- Ability to work within an interdisciplinary setting
