Hospice RN Case Manager (Mon–Fri)
About The Position
Vitality Hospice & Palliative Care is seeking a Hospice RN Case Manager to provide clinical leadership for assigned patients, supported by a strong interdisciplinary team. The role involves conducting comprehensive nursing assessments, developing and managing individualized plans of care, collaborating with various healthcare professionals, educating families, participating in IDT meetings, and maintaining timely documentation. The position is designed to prevent burnout with a manageable caseload, dedicated support staff, and leadership focused on sustainable workloads.
Requirements
- Active Pennsylvania RN License
- Hospice or home health experience strongly preferred
- Strong clinical judgment & communication skills
- Valid driver’s license & reliable transportation
Responsibilities
- Conduct comprehensive nursing assessments (with Admissions Nurse support)
- Develop and manage individualized plans of care
- Collaborate with physicians, LPNs, social workers, chaplains & aides
- Educate families on symptom management and disease progression
- Participate in IDT meetings
- Maintain timely, compliant documentation
