Requirements

  • Registered Nurse (RN) licensed in Pennsylvania, in good standing
  • 1 year of recent hospice experience preferred
  • Strong communication, customer service, and critical thinking skills
  • Travel is an essential function of this position and is required throughout the workday.
  • Employees must possess and maintain a valid driver's license, reliable personal transportation prior to company vehicle assignment, and current automobile liability insurance that meets state minimum requirements.
  • Employees are required to provide updated copies of their driver's license and proof of automobile insurance upon renewal or whenever requested by the Company.
  • As a condition of continued employment in positions requiring driving, employees must successfully pass ongoing motor vehicle record (MVR) monitoring and maintain an acceptable driving record in accordance with Bridges Hospice policy and the Company's insurance requirements.
  • Failure to maintain an acceptable driving record or eligibility to drive on behalf of the Company may affect an employee's ability to perform the essential functions of the position.
  • While Bridges Hospice makes every effort to assign employees to a primary service area or geographic territory, business needs, patient care requirements, staffing needs, emergencies, and other operational circumstances may require employees to travel outside of their assigned territory.
  • Employees are expected to be flexible and willing to provide coverage in other service areas as reasonably requested.
  • Ability to lift/move 50+ lbs and transfer patient weight of 100+ lbs
  • Ability to perform physical job functions (walking, bending, standing, etc.)

Nice To Haves

  • Previous hospice experience strongly preferred.

Responsibilities

  • Performing physical and psychosocial assessments
  • Identifying hospice diagnoses and documenting ongoing eligibility
  • Delivering skilled nursing care
  • Assessing and managing pain and symptoms
  • Ensuring the highest level of comfort and quality of life
  • Educating patients and families on end-of-life processes and rights
  • Developing and coordinating individualized plans of care
  • Initiating referrals and providing ongoing emotional and palliative support
  • Supervising hospice aides per Medicare requirements
