About The Position
Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join their growing team serving patients and families throughout Philadelphia. Requirements
- Active Pennsylvania RN license
- Strong clinical assessment and organizational skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-centered approach
- Ability to work independently and as part of a team
- Valid driver’s license and reliable transportation
- 1 year of experience
Responsibilities
- Manage and coordinate individualized hospice patient care plans
- Perform comprehensive nursing assessments and ongoing patient evaluations
- Provide expert symptom management and compassionate nursing care
- Educate patients and families about disease progression, medications, and comfort-focused care
- Collaborate with physicians, caregivers, and interdisciplinary team members
- Maintain accurate, timely, and thorough clinical documentation
- Provide emotional support and guidance to patients and loved ones
- Advocate for patient comfort, dignity, and quality of life
