Requirements
- Active RN license
- CPR certification
- Valid driver’s license & auto insurance
Nice To Haves
- Hospice experience (preferred, but passion for hospice is key!)
- Experience with HomeCare HomeBase (a plus)
Responsibilities
- Perform comprehensive assessments of patients’ physical, emotional, and psychosocial needs
- Develop and manage individualized plans of care to ensure comfort and dignity
- Guide and educate patients and families through the end-of-life journey
- Collaborate with the interdisciplinary team to deliver holistic care
- Supervise and support Hospice Aides and LPNs/LVNs
- Monitor patient progress and communicate changes to the care team
- Provide compassionate support during critical moments
