Hospice RN Case Manager - Gulf County
About The Position
Big Bend Hospice is expanding into Gulf County and is looking for compassionate professionals to be part of the founding team. This is an opportunity to be one of the first nurses to establish a trusted hospice presence in the community, building relationships, serving local families, and shaping the future of end-of-life care. The role involves coordinating and managing hospice care for patients throughout Gulf County, completing admissions, assessments, and ongoing case management. It also requires collaboration with physicians, hospice aides, social workers, chaplains, and other clinical professionals to help patients and families navigate end-of-life journeys with dignity and compassion.
Requirements
- Florida Registered Nurse (RN)
- Hospice, home health, oncology, long-term care, or strong medical-surgical experience preferred
- Compassionate communicator and patient advocate
- Self-motivated professional who enjoys working independently while being part of a collaborative team
Responsibilities
- Coordinate and manage hospice care for patients throughout Gulf County.
- Complete admissions, assessments, and ongoing case management.
- Collaborate with physicians, hospice aides, social workers, chaplains, and other clinical professionals.
- Help patients and families navigate one of life's most important journeys with dignity, compassion, and expert clinical care.
