About The Position
Hospice Care Partners (HCP) is a Medicare-certified hospice agency based in Irving, Texas. We are a hands-on, growing team focused on comfort, dignity, and responsive support for patients and families. We are expanding into North Dallas and building a strong clinical team under our new Director of Nursing.
Requirements
- Active Texas or compact multistate RN license in good standing
- Sound clinical judgment and comfort working independently in the field
- Strong documentation habits and follow-through
- Dependable, with reliable transportation, a valid driver’s license, and current auto insurance
Nice To Haves
- Hospice or home health experience preferred; strong med-surg or acute care experience considered
- Consolo EMR experience is a plus
Responsibilities
- Complete admission and comprehensive assessments, and update the plan of care through recertification
- Case manage assigned patients: symptom management, ongoing visits, and timely care plan updates
- Support and educate patients and families, including guidance through the end-of-life process
- Participate in weekly IDT meetings and coordinate care across the team
- Communicate with facility staff and referral partners in your territory
- Document accurately and on time in our EMR (Consolo)
- Participate in on-call coverage [rotating weekends]
