About The Position

Ennoble Care is hiring for a full-time Hospice RN Case Manager who resides within or in close proximity to Chester County, PA. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.

Requirements

  • Valid State license to practice as a Registered Nurse in the State of Pennsylvania.
  • 1-2+ years of work experience as a professional nurse, preferably at least one year in hospice care.
  • Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
  • Skilled in nursing practice, able to cope with loved ones/caregivers’ emotional stress and tolerant of individual lifestyles.
  • Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports.
  • Must be a team player who supports an interdisciplinary team concept.
  • Must have a valid driver’s license, a good driving record, the use of a reliable vehicle, and the ability to drive year-round to locations.
  • Must be able to take direction, work independently, and effectively as part of a multidisciplinary team in a positive, professional manner that supports the organization's mission.

Responsibilities

  • Manage assigned patient caseload within territory, including assessing, planning, implementing, and evaluating patient care.
  • Document all notes in the EMR accurately and in a timely manner.
  • Communicate and partner cross-departmentally with attending physicians, hospice staff, and other agencies to coordinate optimal patient care.
  • Educate and coach relevant parties on best practices regarding patient care levels (physical, mental, emotional, etc.).
  • Partner with clinical leadership to manage issues, share professional support, and exchange feedback.
  • Attend patient care conferences and facilitate discussion of caseload issues.
  • Supervise and document the NAC every two weeks or as assigned.
  • Assume the role of associate nurse during off-hours or as requested to fill patient/family needs during the absence of other primary nurses.
  • Provide appropriate support at the time of death and during the bereavement period for family members and loved ones.
  • Participate in Hospice orientation, in-service training, and community programs related to hospice care.
  • Maintain familiarity with agency policies and state and federal regulations governing hospice.
  • Accept other assignments as appropriate.
